Documents you need to bring with you when you visit the Health Center for the first time, please bring the following form of proper current identification:

1. Valid Driver's License
2. Social Security Card
3. Or Birth Certificate
4. Or another form of official identification such as Passport
5. Medi-Cal Card
6. Medicare Card
7. Or Private Insurance Card
8. If you do not have any form of insurance, please bring a current pay stub, unemployment stub, or a letter from your employer for proof of your income and family size so you may qualify for sliding scale fees for the services.

National Health Services, Inc. Community Health Centers accepts Medi-Cal, Medicare, most HMO plans, most private insurances and cash.

They have representatives and outreach workers from Medi-Cal, Healthy Families, and Healthy Kids at their Health Centers to assist you, if you need to enroll in any of these programs.
