Intake procedure:

Call for phone intake. Answering machine operates after hours and on weekends. In 5-7 days a caseworker will contact client directly.


Eligibility:

Must have diagnosis of any chronic disease, including HIV.
Documents required: Social Security card(s), photo ID/driver's license.
