To qualify for a sliding fee discount, please bring the following information with you to register:

1. Proof of income, for every household member.

This may include:
a paycheck stub or
a copy of your tax form reporting your income or
a bank statement if you receive direct deposit

2. Proof of residence (for example, a bill addressed to you).

3. Evidence of health or dental insurance coverage, if any.

4. A Picture I.D.
