Services:

ACUTE DENTAL SERVICES - limited to symptomatic
For age 18+Tuesdays and ThursdaysDoors open for registration and daily sign-up begins at 10:00 am. Most patients are asked to return in the afternoon.

RESTORATIVE DENTAL SERVICES - including fillings
Current patients are scheduled by the dentist for restorative visits.

DENTAL HYGIENE - current patients may schedule visits at front reception desk
Wednesdays only9:00 am and 1:00 pm during Greenville Tech school term schedule (During the fall and spring only.)

PEDIATRIC DENTAL SERVICES
Available on Friday mornings beginning in October at 9:00 am.


Eligibility:


Must qualify as a patient and bring the following eligibility information with you:


Picture ID (valid drivers license, visa, or passport)
Social Security card
Current bill or official correspondence in patient's name, or rental agreement in patient's name. (Documents must be dated within the last month.)
One month of recent paycheck stubs
Food stamp information/print-out (with $ amount and date)
Gross earnings statement from employer on company letterhead
Schedule C of current income tax return for self-employed
Current Social Security benefit letter
Pension records
Housing authority benefit letter
Entitlement letters
Proof of any other form of income
