Dental patients are initially seen in the Clinic by the Nurse Practitioner and then placed on the schedule of one of the dental providers in his/her practice to receive dental care.


To become of patient of Good Samaritan Health Clinic, you must be:

  • Resident of Cullman County,

  • Have two forms of identification

  • Provide proof of all household income.

  • Not be covered by Medicare, Medicaid, Veterans or private insurance.

  • All applicants must provide a Medicaid denial letter.
