    The staff is great except for their secretary. The secretary is extremely difficult to deal with. We had a routine appointment scheduled and rescheduled three times. When I showed up with our children for the appointment, the secretary laughed and said that we were a day early. Apparently, she had rescheduled us without letting us know. This had happened with this office before. When we finally had this appointment which had been rescheduled four times, we then waited forty minutes to be taken back. Ridiculous.

General dentistry. The Crescent Park Dental Clinic provides full service dental care by experienced Dental Practitioner, Theodore Rockwell, DMD, public health dental hygiene practitioners and an expanded functions dental assistant.

Patient Eligibility:
Residents of Forest and Warren Counties and anyone referred by a Warren County-based physician.

Fees:
Accepts Medical Assistance and MED Plus; a sliding-fee scale, based on income and family size, is available for the uninsured.
