Mondays through Thursdays 9:00am - 5:00pm.

By appointment only.

Eligibility Screenings:

Tuesdays, Wednesdays 1:30pm 3:30 pm.

Must provide proof of low- income status.

Must have lived in Forsyth, Stokes, or Davie counties for at least the last three months.

Cannot be eligible for state or federal sponsored healthcare insurance, including Veterans Administration, Medicaid, or Medicare.
